Request an Itemized receipt and/or letter of medical necessity

SoMD Wellness can provide our clients with itemized receipts which can be used for reimbursement or submitting claims to insurance. Please note that SoMD Wellness is NOT affiliated with any insurance. Receipts are not available until after an appointment has completed and can take up to one week to complete.

If you have a form that must be filled out, please submit to us via email. Forms/documents that require to be filled out will have a charge of $10-$20 and won’t be available to client until payment is received.

Any requests for documentation will be sent to the client via the patient portal.
